Account & Data Requests
Customers and authorized USHS Ops users can ask us to review, correct, delete, or de-identify eligible information. This page explains how to make a request and what happens next.

What to include
- Your full name and the email address or phone number tied to the record.
- Whether the request concerns a customer record, a job application, or a USHS Ops account.
- The specific information you want reviewed, corrected, deleted, or de-identified.
- Enough detail for us to locate the record without sending passwords or other account secrets.
Identity verification
Before acting, we verify that the requester is the person the information concerns or is authorized to act for that person. We may ask for additional information that reasonably matches the account or service record. Do not send a password, one-time code, payment card number, or government identity document unless we provide a secure, request-specific way to do so.
USHS Ops accounts
USHS Ops accounts are provisioned by administrators. The app currently does not offer instant self-service account deletion. An administrator can deactivate access while a verified request is reviewed, but deactivation alone does not erase historical messages, calls, customer records, task history, or audit records associated with the account.
What may be deleted or retained
After verification, we review the information connected to the request. Eligible information may be corrected, deleted, or de-identified. We may retain records that are reasonably needed for legal, tax, accounting, payroll, employment, contract, security, customer-service, audit, fraud-prevention, or dispute purposes, or to preserve the integrity of shared operational history. If we cannot complete a requested action, we will explain the applicable limitation when we respond.
